I got up to run my 21 mile benchmark yesterday (Sunday) at 6am since I had to work Saturday morning-I got all dressed, felt fine, headed out the door and ran a total of a mile and a half before turning around and walking home due to throbbing pain in my knee-the outside of my knee to be exact, where I assume my IT band would be. I sat on the couch all day watching football and icing my knee, got up again this morning (Monday) and as soon as I stepped out of bed I can feel that I won't be running this morning.
I didn't feel any pain all week-I've only experienced this IT Band pain once before and that was after my 30K-and that lasted only a few hours-I was fine the next day. Now I did miss 2 weeks following the 30K with a bad flu, but ran the 9 miles last Saturday and have been on track with my weekly runs...now I'm concerned that I'm falling behind with less than 3 weeks to go to the marathon.

IT Band Syndrome is notorious this time of the training program. The term syndrome indicates "more than one factor contributing to the disorder". This means your knee is hurting, but is it due to flexibility of the glutes and hip flexors, weakness of the glute medius, adductors, or arches of the feet.....is there an anatomical leg length inequality....Genu valgus or varus knees, large Q Angle of the Hip...etc, etc, etc.....regardless, your knee hurts.

Since this is your first time experiencing it in a while, that may be a good indication of a mild irritation. Most likely your glutes and hip flexors are in need of stretching...but don't stop there, stretch your quads, hamstrings, and calfs as well. Ice the area that hurts. As far as activity goes, don't do what hurts. If it hurts to run...don't. Cycling, the Elliptical, and water running are all great substitute activities.

It's tough to answer your question without evaluating the issue and finding what to fix. It’s probably reasonable to say the marathon may be uncomfortable for you...the goal is to minimize the discomfort and maintain your function/ability.
